Jacob attended Wellington Point State primary school and Cleveland high
Jacob was a wonderful sportsman ..he excelled in soccer and futsal
Jacob passed away 16th June 2005 after a 20 month battle with acute myeloid Leukaemia
he is missed with every beat of our hearts
